Parent/Guardian Consent (if under 18 years old)
If the applicant is under the age of 18, this section must be completed by a parent or legal guardian. As the parent or legal guardian, you acknowledge and give your full consent for your child to engage with ROR Sports Management and participate in the services offered. By providing this consent, you are agreeing to all terms, conditions, and agreements on behalf of your child, including the collection, storage, and use of their personal data.
You also acknowledge that you have read and understood the Terms and Conditions and that you accept the responsibilities and obligations associated with managing your child's athletic career through ROR Sports Management. This includes overseeing your child's participation in the programs and services we offer, as well as ensuring that all actions are in compliance with our policies.
As the consenting parent or guardian, you confirm that you have the legal authority to make decisions on behalf of your child and to enter into this agreement. You also understand that by providing this consent, you are supporting your child's athletic ambitions and partnering with ROR Sports Management to help them achieve their career goals.
